After School Dismissal Plan and Changes Form

Please submit the After School Dismissal Plan and Changes Form whenever there is a one day change or a permanent plan change for your student. To keep our students safe it is important to know where they are going at the end of the school day.

* Please select Permanent Plan when your student is starting a new weekly routine.

* Please select One Day Change for each day that differs from the permanent plan. One Day Change can be submitted in advance, but it must be received by 10:00 AM on the day the change is for.

* If another authorized adult is picking up your student, select "Other Adult" and list the name and phone number of that person.

* 4th & 5th grade families have the option to select “self-release” for their child’s dismissal plans at 2:30. This allows students to independently follow the dismissal plans you shared with them. This can be a nice way to give your older child more responsibilities as they prepare for middle school.
